Admission Process

1
Tour and Application

Preferably, a tour of the school will have been given before an application has been submitted.  If not, a tour is strongly recommended as soon as possible to gain familiarity and understanding of Elizabeth Academy before moving forward with the admissions process.

To begin the application process, complete and return this application form for the Garfield School campus or this application form for the Elizabeth Academy campus. We also require any evaluations, school records and any other information that you feel will help us get better acquainted with your child.  Once your application process is complete and all forms have been submitted, we will contact you to schedule your family interview and placement screening visit.

2
Placement Screening (and Developmental Screening, if necessary)

The placement screening is for all students and intended for us to get to know you and your child and for you to ask any questions you may have about our programs. It is also a time for us to make an informal assessment of your child’s needs so that we may place him/her in the optimal classroom environment. The placement screening will be a $50 fee (payable via cash, check or credit card).  A developmental screening may also be scheduled at the same time if you are aware of, or concerned about the possibility of any areas of needed support your child might have.  If during the initial placement screening process, we find that an additional screening will be necessary to fully evaluate your child’s needs, a developmental screening will be coordinated either at the same time or on a different date.  The developmental testing measures a broad spectrum of possible needs (Speech and Language, Occupational therapy, Physical therapy and Social/Emotional) and is administered by qualified experts in these areas.  There is a $200 additional fee for developmental testing.

3
Enrollment

Open enrollment for new families begins on February 21st. If your child is offered a placement, you will have two weeks to return your enrollment contract, non-refundable commitment fee and materials and activity fee to secure your child's space.

2
Payment

Enrollment is for the entire academic year and we offer four payment options:

Full Year Payment: If you choose to pay the entire annual tuition in one payment, you will receive a three percent (3.0%) discount as long as your tuition is paid in full by May 20, 2017. This discount applies only to families who do not receive a Carson Smith Scholarship or tuition assistance discount or any other scholarship adjustment. Tuition in full is paid via check or credit/debit card (2.5% convenience fee for this option) or through FACTS Management Company (“FACTS”) by one of the following methods: (1) automatic withdrawal from your bank account – FACTS does not charge a fee for this option; or (2) credit card - FACTS will assess a 2.85% fee for this option. Payment is due within 10 business days of returning the enrollment contract.

Two Payments: Tuition in two installments (50% due in May 2017 and 50% due in November 2017) is paid through FACTS Management Company (“FACTS”) by one of the following payment methods: (1) automatic withdrawal from your bank account; or (2) credit card - FACTS will assess a 2.85% fee for this option. FACTS charges an annual fee of $10 per household for the two installment option and you must sign up on FACTS within 10 days of returning the enrollment contract. Failure to do so will result in a $100 fee and could jeopardize your child’s enrollment.

Monthly Payment: Annual tuition is also payable in twelve (12) equal monthly installments. The first tuition payment will be due either on May 5th or May 20th, 2017 (depending on which withdrawal option you choose). The remaining eleven (11) installments are due on the 5th or 20th day of each month from June 2017 – April 2018. Monthly tuition is paid through FACTS Management Company (“FACTS”) through: (1) automatic withdrawal from your bank account; or (2) credit card - FACTS will assess a 2.85% fee for this option.  FACTS will charge an annual $45.00 fee per household for each school year that you elect to pay using the monthly installment payment program and you must sign up on FACTS within 10 days of returning the enrollment contract. Failure to do so will result in a $100 fee and could jeopardize your child’s enrollment.

Bi-Monthly Payment: Annual tuition is also payable in twenty-four (24) equal monthly installments. The first tuition payment will be due on May 5th, 2017. The remaining twenty-two (22) installments are due on the 5th and 20th days of each month from June 2017 – April 2018. Failure to do so will result in a $100 fee and could jeopardize your child’s enrollment.
